Have you lost or damaged your 12 pin or 14 pin flex cable that connects the USB PCB to the Motherboard? Just get yourself a replacement and bring back the USB port functionality back.
Please be sure to check which pin count you have as there are 2 variants of the cable used in the various Playstation 4 controllers.
